ABSTRACT

Objective:To explore the application value of humanistic literacy and clinical practice ability training in postgraduate courses of geriatrics.Methods:Postgraduates in Zhejiang University School of Medicine were taken as research objects. The teaching curriculum combining humanistic literacy and clinical practice ability training was set as the course of experimental group, and the teacher-led teaching course was set as the course of control group. The questionnaire survey was conducted to the evaluate the teaching effect among the postgraduates from three aspects: classroom situation, knowledge application and ability training. SPSS 17.0 was used for t-test. Results:The teaching of combining humanistic literacy and clinical practice ability training in classroom atmosphere, participation, teacher-student communication, helping memory, knowledge comprehension, knowledge application, collaboration ability, expression ability, organizational ability, knowledge acquisition ability, and autonomous learning ability scores of the experimental group were (3.96±0.20), (3.92±0.28), (3.75±0.44), (3.92±0.28), (3.96±0.20), (3.96±0.20), (3.83±0.38), (3.92±0.28), (3.79±0.41), (3.88±0.34), and (3.83±0.38), which were better than those of the control group, especially in classroom atmosphere, participation, knowledge application, expression ability, and autonomous learning ability.Conclusion:The combination of humanistic literacy and clinical practice ability training can significantly improve the classroom atmosphere of geriatric postgraduate courses, enhance students' medical humanistic literacy, familiarize students with the core skills of geriatrics, and improve students' ability to express and learn independently.

ABSTRACT

Objective To introduce a new posterior-anterior approach used to establish tibial tunnel for all-inside posterior cruciate ligament (PCL) reconstruction and evaluate its short-term therapeutic effects.Methods We retrospectively analysed the 15 patients (15 knees) with PCL injury who had been treated at Department of Orthopaedics,Zhejiang Provincial People's Hospital from January 2017 to January 2018.They were 9 men and 6 women,with a mean age of 46.3 years (from 23 to 60 years).Of them,8 left and 7 right sides were injured.The intervals from injury to surgery averaged 33.6 days (from 4 to 120 days).Their injury was complicated with medial meniscus tear in 3 cases,lateral meniscus tear in one case,lesion of medial collateral ligament in 2 cases and lesion of lateral medial collateral ligament in one case.After preoperative physical examination and MRI confirmed complete PLC tear in all of them,they were treated by arthroscopic all-inside PCL reconstruction in which a new posterior-anterior approach was used to establish the tibial tunnel.Lysholm scoring and International Knee Documentation Committee (IKDC) scoring were used preoperatively and postoperatively to evaluate the knee functions.Results Their follow-ups averaged 8.3 months (from 6 to 12 months).Operation time averaged 55 minutes (from 45 to 70 minutes).All the patients got rid of the knee instability after surgery.All their posterior drawer tests and sag signs turned to be negative.At the final follow-up,their range of motion of the knee ranged from 110° to 130° (average,118°);their average IKDC score was 88.1 ±3.3 (from 82 to 93),significantly higher than the preoperative value (52.3 ±2.1) (P < 0.05);their average Lysholm score was 90.6 ± 3.1 (from 84 to 96),significantly higher than the preoperative value (43.1 ± 2.3) (P < 0.05).Conclusions This new method can avoid neurovascular damage and overcome the killer turn because it provides adequate exposure of the tibial PCL footprint and accurate placement of the tibial tunnel.It is also very convenient because the new posterior-anterior approach can be used to establish the tibial tunnel in all-inside PCL reconstruction without special tools like tibia1 PCL guide and retrograde drill.Its short-term clinical results have proved to be satisfactory.

ABSTRACT

Objective To investigate the short-term clinical efficacy of arthroscopic reconstruction of posterior cruciate ligament (PCL) via double posteromedial portals.Methods A retrospective case series study was performed on the clinical data of 29 patients with PCL injury from January 2013 to January 2018 admitted to Xuancheng People's Hospital of Anhui province.There were 15 males and 14 females,with an average age of 49.6 years (range,27-61 years).There were 13 patients with left knee injury and 16 patients with right knee injury.The combined injuries included five patients with medial meniscus tear,seven patients with lateral meniscus tear,one patient with medial collateral ligament injury,and two patients with lateral medial collateral ligament injury.The average duration from injury to surgery was 14.1 days (range,5-97 days).All patients received arthroscopic PCL reconstruction surgery using double posteromedial portals.The operation time and intraoperative blood loss were recorded.The posterior drawer test,reverse Lachman test,posterior translation,and Lysholm knee score scale were used to evaluate knee function preoperatively and 6 months after operation.The complications were recorded.Results All patients were followed up for 6-22 months (mean,12 months).The operation time was (63.3 ±9.5) minutes,and the intraoperative blood loss was (48.9 ± 22.8)ml.The posterior drawer tests and reverse Lachman tests of all patients turned to be negative 6 months after surgery.Preoperatively,the posterior translation and Lysholm score were (11.2 ± 3.3)mm and (42.7 ± 12.2)points,respectively.At 6 months postoperatively,the posterior translation and Lysholm score were (2.2 ± 0.5) mm and (86.3 ± 9.0) points,respectively (P < 0.05).No complication such as infection and neurovascular injury occured.Conclusion The technique of arthroscopic PCL reconstruction surgery using double posteromedial portals has the advantages of good knee joint stability,less tibial posterior translation,and fast function recovery with satisfactory short-term efficacy.

ABSTRACT

Objective To explore the impact of systematic retroperitoneal lymphadenectomy (SL) in patients with advanced ovarian cancer. Methods The data on 188 patients with FIGO stageⅢC epithelial ovarian cancer were retrospectively analyzed. All the patients underwent primary debulking surgery and then received platinum-based chemotherapy. The patients were divided into SL group and non-systematic lymphadenectomy (USL) group. Progression free survival (PFS) and overall survival (OS) times were analyzed. Results PFS and OS were higher in SL group than in USL group (36 versus 17 months and P = 0.022; 58 versus 52 months and P = 0.041). Univariate analysis showed that both SL and optimal debulking surgery were the independent prognostic factors , but multivariate analysis indicated that optimal debulking surgery was the only independent prognostic factor. However , for 143 patients without enlarged retroperitoneal lymph nodes , multivariate analysis showed that either SL or optimal debulking surgery were the independent prognostic factor (P = 0.040 and P = 0.014). Conclusions Optimal debulking surgery is helpful in improvement of the survival in patients with advanced ovarian cancer. For patients without enlarged retroperitoneal lymph nodes, SL still has a positive effect on the prognosis.

@#Objective To evaluate the damaged level of auditory processing and diagnose the type of language impairment in an auditory agnosia patient using Psycholinguistic Assessment in Chinese Aphasia (PACA), and predict the outcome of the language function. Methods A patient with a bilateral damage involving the temporal lobe could physically hear the sounds, but was unable to recognize or differentiate between the sounds. Speech and language evaluations were taken with PACA 4 weeks, 8 weeks and 3 months after the onset of disease. Results At 4 weeks, the correct rates were 2.5% and 95.0% in spoken word-picture matching and written word-picture matching (P<0.001), and were 5.6% and 80.6% in spoken semantic knowledge and written semantic knowledge (P<0.001). The scores of auditory input processing function score were low, including phoneme discrimination, spoken word matching with minimal differences, environmental sounds identification. The correct rates were 67.5% in oral picture naming, 0 in word repetition, 70.0% in word reading, 0 in dictation, 100.0% in directly copy writing. At 8 weeks and 3 months, there was complete recovery in written word- picture matching (both 100%) and written semantic knowledge (97.2%, 100%), and improvement in oral picture naming (87.5%, 90%), word repetition (87.5%, 97.5%), picture name writing (77.5%, 87.5%) and copy writing (both 100%) (P<0.05). But no change was found in auditory input processing function examinations, spoken word-picture matching (5.0%, 7.5%), spoken semantic knowledge (2.8%, 5.6%), word repetition (0, 3.3%) and dictation (both 0). Conclusion The patient was diagnosed as auditory agnosia with the inability to distinguish sounds. PACA can well identify the impaired level of auditory comprehension disorder and the outcome of language function recovery for aphasics.

ABSTRACT

Objective To evaluate the feasibility and outcomes of different surgical approaches on the basis of sentinel lymph node biopsy (SLNB) in treating early-stage vulvar cancer, and discuss the proper strategy for individualized treatment. Methods The medical charts of patients with early-stage vulvar cancer treated in Sun Yat-sen University between January 2004 and December 2013 were retrospectively collected. A total of 74 patients who received sentinel lymph node(SLN)detection in primary surgery were enrolled (average age 55). The surgical approaches contained SLNB, inguinal lymphadenectomy (IL), and extensive vulvectomy. The SLN were examed on intraoperative frozen sections. The treatment protocols, lymphatic metastasis, postoperative recovery condition, recurrence and survival data were collected and analyzed. Results At least one SLN was successfully detected in 68 (92%,68/74) patients. SLN were positive in 21 patients, of whom 12 (group A) underwent bilateral IL, and 9 (group B) received radiotherapy without performed IL. SLN were negative in 47 patients, of whom 26 (group C) underwent bilateral IL and one of them had a non-SLN metastasis, and 21 (group D) were advised to follow-up. The coincidence of pathological results between frozen and paraffin sections was 100%. The sensitivity and specificity of SLNB for diagnosis of lymph node metastasis were 95% and 100%, respectively. A total of 44 complications happened in patients underwent SLNB and IL (group A and C), including 16 poor wound healing, 14 lymphedema, 8 lymphatic fistulas, 3 phlebothrombosis and 3 infections. There were no complications happened in patients underwent SLNB alone (group B and D), among whom the operation time, bleeding amount, and hospital stay were also significantly less than those in patients underwent SLNB and IL. The median follow-up time was 41 months and the 3-year overall survival rate was 85% in the whole series. Recurrences were observed in 11 patients and 9 of them died of the tumor with the median survival time of 15 months. In patients with positive SLN (group A and B), the 3-year overall survival rate was 58% with 8 patients died of the disease, including 4 in group A and 4 in group B. In patients with negative SLN (group C and D), the 3-years overall survival rate was 97% with one patient in group D died of the tumor, and significantly higher than that of patients with positive SLN (P=0.003). The 3-year overall survival rate was significantly difference. In univariate analysis by log-rank test showed that, neither in patients with nor without SLN metastasis the prognosis differed with respect to surgical approaches (group A vs B, P=0.709;group C vs D, P=0.253). Univariate analysis by log-rank test showed that, lymph node metastasis, pathological grade, depth of invasion, and tumor location could significantly affected survival (P0.05). Multivariate analysis showed that lymph node metastasis (RR=21.57, 95%CI:2.68-173.10, P=0.002) and tumor location (RR=7.85, 95%CI:1.79-34.50, P=0.024) were the independent factors for overall survival. Conclusions Lymph node metastasis is an independent prognosis factor for patients with early-stage vulvar cancer. SLNB could accurately diagnose the status of lymph nodes and help to decide subsequent treatment. The omissions of IL in patients with negative SLN avoid surgical morbidity and shorten postoperative recovery period without an increased risk of recurrence.

ABSTRACT

Objective To compare the curative effect of 360° arthroscopic denervation and routine arthroscopic debridement in treatment of knee osteoarthritis.Methods A total of 85 patients were included in the study after excluding the 5 follow-up dropout among the 90 patients with knee osteoarthritis treated between May 2006 and May 2011.Subsequently,the odd and even quality of the mantissa of admission number was used to determine the groups:Group A,42 patients underwent routine arthroscopic debridement; Group B,43 patients underwent arthroscopic debridement plus 360 degree denervation.Traumatic and degenerative arthritis occurred in 30 and 55 patients respectively.There were 34 male and 51 female patients whose mean age was 63 years (range,52-73 years).Western Ontario and McMaster University Osteoarthritis Index (WOMAC),MOS item short from health survey (SF-36),and visual analogue score (VAS) were compared between the two groups before and after operation.Results WOMAC score and SF-36 score of the two groups improved after surgery.At postoperative 6 months,12 months,and 24 months,WOMAC in Group B scored 81.0,78.5,and 82.4 respectively,lower than 86.8,83.8,and 91.7 in Group A (P < 0.05) ; SF-36 in group B scored 47.4,46.3,and 44.4 respectively,higher than 43.9,41.4,and 39.5 in group A (P < 0.05).At postoperative 1 week,VAS of group B was 1.6 points,lower than 2.6 points in group A (P < 0.05).There was no statistical difference between the two groups in postoperative swelling of joint (P > 0.05).Conclusions According to the area involved in knee osteoarthritis and characteristics of innervation of knee joint synovium,arthroscopic 360° denervation provides thorough debridement and removes as many innervation of knee joint synovium.Arthroscopic 360° denervation is superior to the routine arthroscopic debridement in improvement and duration of the symptoms.

ABSTRACT

Objective To investigate the characteristic of surgical treatment of burns in pediatrics patients. Methods From January 2006 to March 2012,47 pediatrics patients and 244 adult patients were received the surgical treatment in our hospital. The clinical data of two groups were analyzed retrospectively and compared. Results The female ratio in pediatric patients was higher than adult group,and almost patients received operation,especially plastic surgery and skin repair,with less surgeries of wound repair and infected surgeries. The preopera-tive physiological index between two groups was no significantly different. Conclusion On the premise of effective preoperative preparation, we could carry out surgical treatment of burns for pediatrics patients positively.

ABSTRACT

Objective To evaluate the feasibility and value of dual-energy perfusion imaging (DEPI) of dual-source CT(DSCT) in the diagnosis of acute experimental pulmonary embolism. Methods Acute pulmonary embolism ( PE ) model was made in 8 New Zealand rabbits, and non-enhanced and enhanced DSCT scans were performed before and after embelization. Postprocessing of image data was made on the workstation, and CT pulmonary angiography ( CTPA ), DEal and fusion images were obtained. The location and number of the emboli were recorded. The rabbits were killed immediately after DSCT scan, the location and number of the lung lobes with pulmonary emboli were evaluated pathologically. Based on the pathological results, the sensitivity, specificity, positive predictive value ( PPV), and negative predictive value (NPV) of CTA, DEal and fusion images for the diagnosis of PE were calculated. Weighted Kappa values were calculated to evaluate the consistency between CTPA and DEal. Results PE model was made successfully in 7 rabbits. Six rabbits with 30 lobes were evaluated with one exception because of the catheter affecting the quality of lung peffusian image. PE was found pathologically in 18 lobar arteries. On DEPI, the region with PE showed low peffusion area comparing with the normal parenchyma and CTPA showed the filling defect within corresponding pulmonary artery or interruption of the artery. The sensitivity, specificity, PPVand NPV of CTPA were 66. 7% (12/18), 100.0% (12/12), 100.0% (12/12) and 66. 7% (12/18), respectively. The Kappa value was 0. 651 indicating moderate correlation with pathology. The sensitivity, specificity, aPv, and NPV of DEal were 88. 9% ( 16/18), 91.7% ( 11/12, 94. 1% (16/17) and 84. 6% ( 11/13 ), respectively. The Kappa value was 0. 795 indicating excellent correlation with pathology. Conclusion Dual-energy lung perfusion imaging of DSCT can display the blood distribution of rabbit's lung and has a high sensitivity for the diagnosis of acute pulmonary embolism.

ABSTRACT

Objective To evaluate the diagnostic accuracy of digital subtraction bone removal CT angiography (DS-BR-CTA) and dual energy bone removal CTA (DE-CTA) compared with conventional and rotational DSA. Methods A total of 264 patients underwent 3D-DSA were enrolled, 190 of them underwent also DS-BR-CTA, while 74 underwent DE-CTA as well. Aneurysms detected with DS-BR-CTA and DE-CTA were analyzed compared with 3D-DSA on per-patient and per-aneurysm basis. The sensitivity, specificity, positive predictive value and negative predictive value of two methods for diagnosis of aneurysm were calculated. Results Taking DSA as the standard, the sensitivity and specificity of DS-BR-CTA to detect intracranial aneurysms was 96.90% and 98.36% on per-patient basis, while 97.37% and 99.97% on per-aneurysm basis, and the sensitivity and specificity for the detection of aneurysm ≤3 mm was 88.89% and 98.36%. The sensitivity and specificity of DS-BR-CTA to detect intracranial aneurysms was 98.25% and 100% on per-patient basis, 95.52% and 100% on per-aneurysm basis, and the sensitivity and specificity for the detection of aneurysm ≤3 mm was 90.91% and 100%. Conclusion DE-CTA has a comparable diagnostic accuracy with DS-BR-CTA to diagnose intracranial aneurysms, and can be used in the routine workflow for detection of intracranial aneurysms.

ABSTRACT

Objective To determinate the toxic elements in Radix Astragali by ICP-MS. Methods The elements of Cu, As, Cd, Hg, Pb, Al, Cr, Sn, Ba in Radix Astragali were analyzed by ICP-MS. Results There were toxic elements in Radix Astragali. The concentration of Cu, As, Cd, Hg and Pb were not over the limits of Chinese Pharmacopoeia, but the concentration of Al, Cr and Ba were over the levels of other elements obviously. Conclusions There are difference of residues of the 9 kinds of toxic elements. It’s necessary to establish the determination methods of toxic elements (especially Al, Sn, Cr and Ba) in Chinese herbal medicines and the limits of the related toxic elements.

ABSTRACT

Objective To study the quality standard of Anshen Jiannao Mixture. Methods Radix ophiopgonis, Radix et Rhizoma Salviae Miltiorrhizae, Fructus Lycii and Radix et Rhizoma Ginseng were identified by TLC. HPLC was used to determine the content of Schizandrol A on C18 (250 mm?4.6 mm, 5 ?m) ODS column. The mobile phase was consisted of methanol-water (55∶45). The flow rate was 1.0 mL/min. The detection wavelength was at 250 nm. Results The characteristic spot of Radix et Rhizoma Salviae Miltiorrhizae, Fructus Lycii and Radix et Rhizoma Ginseng could be easily examined by TLC. The calibration curve of Schizandrol A was linear in the range of 0.121 8~1.218 ?g, r=0.999 9. The precision RSD was 0.22% (n=6). The reproducibility RSD was 1.1% (n=6). The average recovery rate of Schizandrol A was 97.34% (RSD=1.7%, n =6). Conclusions The method is easy, sensitive and specific with good reproducibility. It can be used to control the quality of Anshen Jiannao Mixture.

ABSTRACT

Objective To establish a method for the determination of silybin in Fufang Yiganling tablet. Methods RP-HPLC was used and the mobile phase was consisted of methanol-water-glacial acetic acid (48∶52∶1). The detection wavelength was 286 nm. Results The calibration curve of Silybin was linear in the range of 0.238 0~2.380 ?g, linear equation:Y=2.146?106X +1.818?103, r=0.999 9, with average recovery of 98.17% (RSD=1.3%). Conclusions The method is simple, accurate and with good reproducibility. It can be used to control the quality of silybin in Fufang Yiganling tablet.

Objective:Rudimental contrast media in the subclavian vein often influence the display of carotid on dual-source computer tomography angiography.This article aimed to investigate this influence,and find a best solution.Methods:One hundred patients were randomly divided into Groups A(n=52) and B(n=48).The former received traditional imaging,with 65 ml contrast media,4.0 ml/s injection flow and caudocranial scanning direction,while the latter kept all the parameters unchanged except the scanning direction,from caudocranial to craniocaudal.Each group was graded by original axial images,maximum intensity projection(MIP),silhouette images and VR images for vascular delineation of the carotid.Results: The carotid artery was clearly displayed in Group B,while the subclavian vein was unsatisfactorily revealed in Group A,with statistically significant differences in extent scores between the two groups(P

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the roles of microfilaments in the expression of collagenase and TIMP-1 in normal and hypertrophic scar.</p><p><b>METHODS</b>Cell culture and Northern blot hybridization methods were used in this study.</p><p><b>RESULTS</b>After the disruption of microfilaments with cytochalasin B, mRNA contents of collagenase and TIMP-1 both increased significantly in normal and hypertrophic scar fibroblasts.</p><p><b>CONCLUSION</b>The result suggest that the microfilament cytoskeleton may regulate the synthesis and degradation of ECM, which may be a mechanism of gene regulation during wound healing.</p>

Methods An HPLC-UV method was used for determining the contents of MOA and MOB in all samples. Analytical column was Kromasil C18 (250 mm?4.6 mm, 5 ?m). Mobile phase was acetonitrle-water (55∶45) and detection wavelength was 298 nm. The flow rate of mobile phase was 1 mL/min, and temperature was 30 ℃. Results The contents of MOA in Radix Ophiopogonis cropped in Sichuan and Zhejiang Provinces were 0.004 0%-0.009 6%, 0.006 7%-0.013 4%, and the contents of MOB were 0.002 1%-0.006 2%, 0.015 9%-0.028 2%, respectively. The contents of MOA in the extract of Radix Ophiopogonis cropped in Sichuan and Zhejiang Provinces were 0.007 5%-0.008 8%, 0.011 2%-0.012 6%, and the contents of MOB were 0.003 8%-0.005 1%, 0.020 7%-0.023 8%, respectively. Conclusion The contents of MOA and MOB in Radix Ophiopogonis cropped in Zhejiang Province and its extracts are more than those in Sichuan Province and its extrouts. The method can be used for the purpose of the quality control of Radix Ophiopogonis and its extracts.

ABSTRACT

Objective To investigate the principle and its clinical initial application of the dual-energy imaging with the dual-source CT and increase the realization of it. Methods According to the research results of the dual-energy imaging with the dual-source CT and the course of development of the dual-energy imaging to analyze and summarize the basis radiological physics, the principles, the correlation parameters and the clinical initial application of the dual -energy imaging with the dual -source CT. Results The dual -energy imaging with the dual -source CT is different from other common dual-energy imaging, it has its own special feature. Conclusion According to the analysis of the dual-energy imaging with the dual-source CT, greatly increase the realization of the dual-energy imaging with the dual-source CT.

ABSTRACT

Objective To improve the diagnostic accurate rate by studying best values of chest posterior-anterior digital medical images in soft-copy reading. Methods 100 normal adults’ chest posterior-anterior digital medical images in recent 10 days in our hospital were randomly chosen. Five specialists of our department were invited to see these images by soft-copy reading and the best WW/WL values of different parts of the chest they selected were recorded. All the data were analyzed in statistics and the best WW/WL was got. Results The best WW/WL values of soft tissues of chest wall were 2 083/4 840,ribs 2 667/4 908,lung fiber 2 067/4 960,hilum of lung 1 848/5 130,heart and large vessels 2 206/5 438,mediastinum and diaphragm 2 180/5 530,thoracic vertebrae 1 471/5 801,ribs that covered by heart and diaphragm1 428/5 975. Conclusion Different parts in chest poster -anterior digital medical images need special WW/WL in soft-copy reading for better diagnosis of images.
